Please complete all fields in this form, or your RMA request may be delayed. Pressing the submit button will automatically email the request to BatteryDAQ, or you may print and fax the completed form to 484-687-9904. Upon receipt of product, our technicians will evaluate the device and provide a report of the analysis. The device will be repaired or replaced per our hardware warranty policy. Please do not ship your product until you recieve a RMA Number from a BatteryDAQ representative. Returning a product without an RMA number will cause delays in processing.
